The Amgen Entrust Patient Support Program is here to help you get started on AMGEVITA. Once you are enrolled, you will have access to a variety of helpful, confidential, and free services tailored to you.
ENROLMENT
Enrolment is simple! You or your healthcare team can initiate your enrolment over the phone, by fax, and/or email.
If you have not yet enrolled, speak with your doctor, or call 1-877-936-2735 and enrol today or email amgevita@oneenliven.ca.
REIMBURSEMENT NAVIGATION ASSISTANCE
We are here to help you determine your coverage options for AMGEVITA and assist you with any necessary forms. We also offer financial assistance on a case-by-case basis.
TRAINING ON HOW TO INJECT AMGEVITA
We will help coordinate injection training with a healthcare professional for you, your child, and/or a caregiver of your choice.
PRACTICAL INFORMATION AND RESOURCES
We can help you to manage your treatment with AMGEVITA—providing you with accessible, over-the-phone support and additional AMGEVITA resources in the Resources section of this site.
